The Abortion Pill (RU-486)

The Abortion Pill (RU-486)

The Women’s Clinic offers non-surgical abortions with RU-486 often referred to as “the abortion pill” at our Indianapolis location.

A non-surgical abortion is much like a miscarriage early in a pregnancy. Some women consider this method more “natural” than surgical abortions.

Others prefer surgery because it is completed more quickly and causes less discomfort for a shorter period of time.

When can an RU-486 abortion be performed?
RU-486 abortions are performed from the time a woman knows she is pregnant until 63 days from the first day of her last normal menstrual period.

All patients electing a non-surgical abortion have an ultrasound to accurately confirm the length of the pregnancy.

How does an RU-486 abortion work?
Women take two medications one to two days apart. The first is Mifeprex®, also known as RU-486. Mifeprex® blocks one of the hormones (progesterone) that is important for early pregnancies to develop.

By blocking this hormone, the pregnancy dies and the cervix of the uterus begins to soften.

The second medication is misoprostol, a prostaglandin that causes the uterus to contract (similar to labor) and expel the pregnancy. This causes cramps and bleeding.

What are the steps in terminating a pregnancy with RU-486?
When you return after the pre-abortion visit, the physician provides you the first medication (Mifeprex®) which you will take in his presence.

This is a small tablet that is swallowed with water. He also provides your other medications and birth control. We will also give you a Rhogam injection if you are Rh negative.

A day or two later you will take the other medication, misoprostol, 4 small tablets dissolved in the mouth. Usually within two to four hours you will begin to experience very strong cramps and heavy bleeding. This may last an additional two to four hours.

Follow-up Visit – Two weeks later you should return for a followup ultrasound examination to verify that your uterus is empty and that the abortion was complete.

Can any woman have a non-surgical abortion?
There are certain limitations that prevent women from having this type of termination:
  • Women must be less than 64 days in pregnancy as determined by ultrasound
  • Women with an ectopic pregnancy or who have an IUD in place
  • Women who have bleeding disorders or are taking blood-thinners are ineligible
  • Those who have been on long-term corticosteriod therapy or are allergic to Mifeprex® or Cytotec or other prostaglandins
  • In addition, anyone who might be unable to understand the effects of or comply with the treatment should not use the drug
Will medical abortions revolutionize the way abortions are provided?
RU-486 can only be used by women with a pregnancy up to 63 days.

These women tend to like the privacy and control the medical abortion allows them.

We find that many women still prefer the surgical method and the advantages it offers.

Abortion by tablets or pills (Medical abortion)

What is an abortion pill?
Abortion pills or abortion tablets are designed to bring on an abortion for a woman who is less than nine weeks pregnant (that is less than 9 weeks since her last period).

They are designed to be used in abortion clinics. When a woman attends an abortion clinic, her medical history is taken (for example does she take any medicines, or have any other illness that may be affected by these tablets).

Any risk factors (such as an increased likelihood of excessive bleeding) are identified to make sure that the abortion pills would be suitable for her. The clinic will also confirm that the pregnancy is an ongoing single embryo, less than nine weeks’ size, and that it is located in the womb.

If the pregnancy does not fit into this category (for example a tubal/ectopic pregnancy, or a pregnancy of more than 12 weeks’ size) it would be unsafe for the abortion clinic to prescribe the abortion tablets.

In such a situation the abortion clinic would discuss alternative methods of abortion. If suitable, the woman takes the pills at the clinic, with a certain time-interval between doses, if she wishes to bring on an abortion.

At the clinic there is a specialist doctor on-hand to deal with any complications or problems that may arise during this process, such as excessive bleeding or collapse.

Can I get an abortion pill in Ireland and take it at home?
Abortion pills are not legally available in Ireland. In Ireland, it is not legal for anyone to supply or receive a “prescription-only” medicine online.

It is not legal to buy an abortion pill in Ireland online or to get it for free from any source.

A medical practitioner, either in Ireland or abroad, cannot provide an online prescription for the abortion pill, with the intention of it being supplied in or into Ireland.

These medicines will be detained by Customs and handed to the Irish Medicines Board for enquiry.

Abortion pills should be prescribed only after a face-to-face medical consultation in a country where abortion is legal, and taken under medical supervision, where staff can act quickly if complications arise.

What might the side-effects be of taking an abortion pill?
If a person were to buy abortion pills online or from a friend, there is no way of knowing what the tablets might contain (despite what the advertisement says) and what the side effects might be.

The side-effects of abortion tablets may include- excessive bleeding, pain, collapse, diarrhoea and shortness of breath. It could be very unsafe to take these pills. Also the pills may not work properly.

If a woman has excessive bleeding or pain, or other complications after taking an abortion pill, she should seek medical attention straight away from her GP, a family planning clinic or her local hospital.

Medical staff will always help a woman in this situation. Looking after her could be difficult if the staff do not know that a woman has taken these pills or what the pills contain.

It is advisable to bring any information and packaging that comes with such pills to the hospital/GP/Family Planning clinic as it may help the medical staff in their diagnosis and the medical care they can give.

If I attend medical services and say that I have taken an abortion pill, will they take care of me or will they inform the GardaĆ?
A doctor will always look after a woman in this situation. A doctor is not required to report to the GardaĆ that a woman has taken an abortion pill.

Doctors must not breach patient confidentiality except where they deem it appropriate in limited and exceptional circumstances as provided for by the Ethical Guidelines published by the Irish Medical Council, such as

(i) if the person is under 17 years of age and the doctor believes that the patient became pregnant in abusive circumstances; or

(ii) if the person discloses to the doctor that they were subject to abuse during their childhood and if the doctor believes that they or anyone else is at risk of further acts of abuse; or

(iii) by court order, as required by law.  The doctor should inform the patient (or parents/guardians if under 17) of their intention to make the disclosure and the reasons for doing so unless doing so might endanger the patient.

It is preferable but not necessary in these circumstances to obtain the patient’s consent.

The Ethical Guidelines further state that a doctor may only disclose patient information with or without the agreement of the patient when it is necessary to protect the patient or others from serious risk of death or serious harm or if it is in the public interest.

If they do so, they are obliged to consider whether disclosing the information to an appropriate person/body who understands that the information must be kept confidential, without revealing the identity of the patient would achieve the same potential benefits.

Unless there is a particular benefit in revealing your identity, the doctor is obliged to keep the matter confidential.  If the doctor decides to take any action in relation to your case, he/she is obliged to inform you that they are taking this action unless informing you would cause you serious harm.

What is the difference between taking an abortion pill at home and taking it in an abortion clinic abroad?
There are three significant differences between taking the abortion pill at home and taking it in an abortion clinic:
1.) Attending an abortion clinic abroad is legal.  Under Irish law, women have the right to travel for the purpose of getting an abortion. It is not legal to buy the abortion pill in Ireland, or to get it for free.

2.) When a woman attends an abortion clinic, her medical history is taken, the pregnancy size and location is confirmed and any risk factors are identified to make sure that the abortion pills would be suitable for her.  If suitable, she takes the pills at the clinic, with an interval between doses, under a doctor’s supervision in order to bring on an abortion.  This means that if there are any complications, a doctor is on-hand to deal with them.

3.) A woman has no way of knowing if an abortion pill ordered online or obtained from a friend is a “genuine” abortion pill. This pill may not work. There is no way of knowing what these products actually contain and what the side effects might be for her. This is potentially very unsafe.

How It Works

Medical abortion requires two pills. The first, mifepristone, causes the embryo to detach from the uterine wall. Most women don't feel a thing.

Two days later, a pill called misoprostol contracts the uterus to expel the embryo. You can expect heavy bleeding, with clotting and bits of tissue, to last about two weeks.

Crazy fact: The pills are only 95 percent effective. If you're in the unlucky 5 percent, you can do it again or opt to have a procedure.

Why women and teens abort?

Abortion is not a light decision to go through. Many women realize that they have decided to get read off a life that would otherwise live. But there are issues that take them to that decision and for that we have to respect that choice.

Women decide to abort for one or more of the following reasons:

Financial Instability:

Would be mothers realize that their own financial well being is not stable. They struggle for everything that comes into their life, if their own life is such a struggle how about if they add on a new one? Children are extremely expensive creatures.

A baby will be completely impossible to manage when you can not look after your own self.

One other thing it is easier and far better to abort than to dump a leaving child in some sewer or dustbin. It is easier to leave with abortion than with a dumped child.

This fact makes women to abort and get read off a fetus than a leaving person.

Negative Impact on the Mothers Life:

Have a child out of wedlock or outside a marriage is no easy decision. It may lead to the total degradation of the mothers life. It is therefore better for the mother to get an abortion than to go through with the pregnancy because of the consequences.

Relationship problems /unwillingness to be a single mother:

Many would be mothers do not have stable relationships, having a child means being single mothers. It means fending for this child for the rest of its life or to have a permanent stool at the courts to force the father to help. That is if you know who the father is.

It these issues that force women to go through an abortion and survive the consequences.

Other reasons:

Other reasons are medical and other personal issues. But whatever motivates you to have an abortion it is your choice that we have to accept and appreciate. No person has reason to judge you. It is a private choice. In any case where will these people be when you are raising the child. They will be no where and it will be your baby.

It is your choice.

When is it best to Abort?

Abortion done in the first trimester (90% of all abortions) are considered a safe procedure whether it's done by the pill or surgery. First trimester is between 0 to 12 weeks.

Later abortion is just a little bit risky but no more risky than child birth. Late abortions are like labor but all abortion procedures are safer then labor. One reason is because you can control it more.

None of the abortion procedures are considered dangerous if you are of normal health.

When you are aborting you will get cramps so of course and it hurts a bit. But that is as far it gets.

And if you do not get cramps the embryo/fetus can not come out. Pills used in medical abortion are completely safe.

Abortion Procedure

How Abortion is Done

Abortion means ending a pregnancy before the fetus (unborn child) can live independently outside the mother.

If abortion happens spontaneously before 24 weeks of pregnancy, it is called a miscarriage. An induced (or "therapeutic") abortion is caused deliberately in order to end the pregnancy.

Abortions are (most safely) done in the first trimester (before the 12th week of pregnancy).

Pregnancy weeks are counted from the first day of the woman's most recent menstrual period. The type of abortion performed depends on how far the pregnancy has progressed.

There are two procedures used to abort they include medical and surgical abortions.

Medical Abortion

During the first 49 days of pregnancy, a medical termination is possible without surgery.

Medical abortion uses two different drugs, methotrexate or mifepristone, which may be followed by another drug called misoprostol. While these drugs cause an abortion without surgical procedures such as dilation and vacuum, they do take longer to work than surgical abortion but they are perfect and will do the job for you.

These drugs work by blocking progesterone, a crucial hormone during pregnancy. Without progesterone, the uterine lining does not provide food, fluid and oxygen to the tiny developing baby.

The baby cannot survive. A second drug is then given that stimulates the uterus to contract and the baby is expelled.

You may experience nausea, severe cramping, vomiting and bleeding. But the result is an aborted feotus.

Surgical Abortion

Vacuum aspiration (the suction method) is a surgical type of abortion. It is a surgical procedure that involves anesthesia (mild sedation) and can be performed within the first trimester of pregnancy.

During vacuum aspiration, the cervix (opening of the uterus) is gently dilated (widened) about a quarter-inch, a narrow tube is inserted through the vagina and cervix to the uterus, and then pregnancy and contents of the uterine internal lining are vacuumed out.

The procedure takes only a couple minutes and the woman can usually return home later in the day.

Medical abortion

We provide medical (abortion pill) and the vacuum aspiration methods of abortion.

It involves taking a medication called Mifiprex and any other pill that we see fit for you. We require you to get home immediately as the process begins as soon as you have taken the pill.

You will experience cramping and vaginal bleeding at home, much like a miscarriage. A follow-up visit in 1 to 2 weeks is very important to make sure the uterus is empty. This method can only be done very early in the pregnancy.

It is a relatively simple procedure which in and of itself takes less than 10 minutes. We are able to provide this type of abortion anytime in the first trimester of pregnancy, even if you have just barely missed your period.

All of our appointments are individual, personal and private. You will have our full attention.

Many sources give false information about woman's rights and the risk associated with abortion. Medical research confirms that uncomplicated first trimester abortions do not cause future miscarriages, infertility, breast cancer, or depression.

You will be fine within hours. You will be able to great pregnant again, if and when you want to. You will be fine as medical abortion is the safest method of dealing with unwanted-pregnancies.

Abortion With Medicine

Early Abortion Using Medicine

Abortion is a safe and legal way to end a pregnancy. There are two methods of abortion available to women in the first trimester of pregnancy. A medication abortion (also called medical abortion or abortion with pills) involves taking medicines to end a pregnancy.

An aspiration abortion (also called surgical or suction abortion) is a procedure that uses medical instruments in the vagina and uterus to remove the pregnancy.

We are about abortion by medicine.

Medical Abortion

Medical abortion can effectively be used until the 12th week of one's pregnancy.
It is in line with legal time allowed to terminate in South Africa without questions - which is 12 weeks.

The medical abortion procedure uses tablets instead of surgery to interrupt and expel an early pregnancy.

Early pregnancy is best terminated using medicine because the fetus is very small. It is best targeted by medicine. And at this stage all that a woman terminating will see is blood similar to her usual menstrual cycle.

Medical Abortion is not suitable for you if:
  • You are a smoker aged more than 35 years
  • You have a medical condition such as a heart complaint, high blood pressure or diabetes.
  • You suffer from severe asthma or are on long-term steroid treatment.
  • If you are anemic, have a bleeding disorder or are receiving anticoagulants.
  • You have had an operation on your uterus (womb).
  • You have ever had a bad reaction to Mifepristone or Misoprostol.
  • You have an entopic pregnancy (where the fertilized egg is implanted in the tube rather than in the womb).

    2. You should not drink alcohol or smoke once you have taken the medicine for at least two days after you have been given the tablets. You should not take illicit drugs.
3.Do not take medicine for pain relief (e.g panadol, aspirin) during the process of termination. It will interfere with the termination process.

    YOUR FEELINGS AFTER MEDICATION ABORTION

You may have a wide range of feelings after an abortion. Most women ultimately feel relief. Some women feel anger, regret, guilt, or sadness for a little while. Sudden shifts in hormones may make these feelings stronger.

Serious, long-term emotional problems after abortion are about as uncommon as they are after giving birth. You may be more likely to have emotional problems after abortion for certain reasons. Some of these reasons are

having a history of emotional problems before your abortion
having important people in your life who aren't supportive of your decision to have an abortion
having to terminate a wanted pregnancy because your health or the health of your fetus is in danger

If you want to talk with someone after an abortion, abortion providers can talk with you or refer you to a licensed counselor or to support groups.  You could also contact Exhale, a free, after-abortion talkline, that provides confidential and nonjudgmental emotional support, information, and resources for women who have had abortions.
GETTING YOUR PERIOD AFTER MEDICATION ABORTION

Abortion begins a new menstrual cycle. You should have a regular period in 4 to 8 weeks.
SEX AND BIRTH CONTROL AFTER MEDICATION ABORTION

Many health care providers recommend that you not have vaginal intercourse or insert anything except a tampon into your vagina for one week after the abortion.

You can get pregnant very soon after an abortion. So, birth control is an important and common concern for many women after abortion. Talk with your health care provider about getting a birth control method that's best for you.

Why Do Women Choose the Abortion Pill

Why Do Women Choose the Abortion Pill

It can be done early — women can begin treatment as soon as they know they are pregnant.
It's private — women may complete the process of abortion at home.
        There's usually no anesthesia.
Many women feel it's more "natural" — they feel it is more like miscarriage.
Women may feel more in control — many feel it is less invasive.

Nearly all women who have used the abortion pill would recommend the method to a friend.

How Safe Is the Abortion Pill

Most women can have a medication abortion safely. But all medical procedures have some risks, so safety is a concern.

    Rare, but possible risks include

an allergic reaction to either of the pills
incomplete abortion — part of the pregnancy is left inside the uterus
        failure to end the pregnancy
        infection
        blood clots in the uterus
        undetected ectopic pregnancy
        very heavy bleeding

Most often, these complications are simple to treat with medicine or other treatments.

    Abortion pills http://www.drgracewomensclinic.co.za/abortion-pills.html
    Womens Clinic http://www.drgracewomensclinic.co.za
Contact Dr Grace Womens Clinic http://www.drgracewomensclinic.co.za/contact.html

In extremely rare cases, very serious complications may be fatal. The risk of death from medication abortion is much less than from a full-term pregnancy or childbirth.

Serious complications may have warning signs. Call your health care provider right away if at any time you have

heavy bleeding from your vagina and are soaking through more than two maxi pads an hour, for two hours or more in a row
clots for two hours or more that are larger than a lemon
severe abdominal pain or discomfort that is not helped by medication, rest, a hot water bottle, or a heating pad
chills and a fever of 100.4°F or higher after the day of the procedure
nausea, vomiting, and/or diarrhea that lasts more than 24 hours
an unpleasant smelling discharge from your vagina
signs that you are still pregnant

You should start to feel better each day after the abortion. Feeling sick — having abdominal discomfort, diarrhea, nausea, vomiting, or weakness — more than 24 hours after taking misoprostol could be a sign of serious infection. Contact your health care provider right away if you have any of these symptoms. Do not wait until your scheduled follow-up.

You may need another visit with your provider. Rarely, women need vacuum aspiration or hospitalization. Take your medication guide with you if you need to visit an emergency room, a hospital, or a health care provider.

The abortion pill may not be right for all women. You shouldn't use the abortion pill if you

are more than 63 days — nine weeks — pregnant
are not willing to have an aspiration abortion in the unlikely case that the medicines do not end your pregnancy
        cannot come to follow-up appointments
do not have access to a telephone, transportation, and backup medical care
have a known or suspected molar pregnancy — one in which the placenta develops abnormally
have severe adrenal gland, heart, kidney, or liver problems
take any medicine that should not be combined with the medications used in medication abortion — mifepristone or misoprostol
take anti-clotting medication or have a blood-clotting disorder
currently have an IUD. You will need to have it removed before having a medication abortion.

Talk with your health care provider to find out if medication abortion is likely to be safe for you. If medication abortion isn't right for you, don't worry. Another abortion method is likely to be safe for you.
    BREASTFEEDING DURING MEDICATION ABORTION

A little bit of misoprostol will be present in breast milk after you take it. If you are breastfeeding, the misoprostol may cause your infant to have diarrhea. Let your health care provider know if you are breastfeeding so you can work out the best plan together.

What Happens During a Medication Abortion?

It's common for women to be nervous about having a medication abortion — or any other medical procedure. But many of us feel better if we know what to expect. Your health care provider will talk with you and answer your questions. Here's a general idea of how it works and what to expect.
Before taking the abortion pill, you will need to
    • discuss your options
    • talk about your medical history
    • have laboratory tests
    • have a physical exam. This usually includes an ultrasound.
    • read and sign papers
You will also be given a medication guide, instructions, and other information to take home with you, including a 24-hours-a-day, seven-days-a-week telephone number you can call if you have any questions or concerns.
Medication abortion is a process that begins immediately after taking the first pill.
    There are three steps:

    STEP ONE — MIFEPRISTONE

Your health care provider will give you the first medicine at the clinic. You will also be given some antibiotics.
Mifepristone works by blocking the hormone progesterone. Without progesterone, the lining of the uterus breaks down, and pregnancy cannot continue.

    STEP TWO — MISOPROSTOL

You will take a second medicine — misoprostol. It causes the uterus to empty.
You'll take the second medicine 24-48 hours after taking mifepristone. Your health care provider will give you instructions on how and when to take the second medicine.
The second medicine — misoprostol — will cause you to have cramps and bleed heavily. Some women may begin bleeding before taking the second medicine. But for most, the bleeding and cramping begin after taking it. It usually lasts a few hours. You may see large blood clots or tissue at the time of the abortion.
More than half of women abort within four or five hours after taking the second medicine. For others, it takes longer. But most women abort within a few days.
It's normal to have some bleeding or spotting for up to four weeks after the abortion. Only use pads for bleeding after an abortion.

    HOW DOES MEDICATION ABORTION FEEL?

For most women, medication abortion is like an early miscarriage. It is normal for you to have bleeding and cramping. You might also
    • feel dizzy
    • feel strong cramps
    • feel nauseous or vomit
    • have diarrhea
    • feel temporary abdominal pain
    • have temporary mild fever or chills
    • Acetaminophen (like Tylenol) or ibuprofen (like Advil) can reduce most of these symptoms. Do not take aspirin.
You may feel more at ease if you have a trusted loved one with you during the abortion.

    STEP THREE — FOLLOW-UP

You will need to follow up within two weeks. Follow-up is important to make sure your abortion is complete and that you are well. You will need an ultrasound or blood test.
In the unlikely event that you are still pregnant, your health care provider will discuss your options with you. It's likely you will need to have an aspiration abortion if the medication abortion did not end the pregnancy.

It's common for women to be nervous about having a medication abortion — or any other medical procedure.

But many of us feel better if we know what to expect. Your health care provider will talk with you and answer your questions. Here's a general idea of how it works and what to expect.

Before taking the abortion pill, you will need to

        discuss your options
        talk about your medical history
        have laboratory tests
have a physical exam. This usually includes an ultrasound.
        read and sign papers

You will also be given a medication guide, instructions, and other information to take home with you, including a 24-hours-a-day, seven-days-a-week telephone number you can call if you have any questions or concerns.

Medication abortion is a process that begins immediately after taking the first pill.

    There are three steps:
    STEP ONE — MIFEPRISTONE

Your health care provider will give you the first medicine at the clinic. You will also be given some antibiotics.

Mifepristone works by blocking the hormone progesterone. Without progesterone, the lining of the uterus breaks down, and pregnancy cannot continue.

Having a child should be a choice. You should not be forced into child birth when you are a child yourself, if your finances do not allow, if you do not have a committed partner or any other reason unique to you.

Having and looking after a child is very serious business that you should only and only join if you are ready.

In many countries of the world, women essentially take these pills at home and have the miscarriage, or the abortion, at home. And I just want to note here, that this approach has been shown to be highly safe and effective.

The effectiveness rates, and what we mean by effectiveness is that the woman safely and effectively has the abortion process or the miscarriage process at home with no need for any surgical intervention, are about 95% to 98% , depending on the actual gestational age.

Now, that being the case, you can understand for the vast majority of women, they really don’t need to come in to the office for a follow-up.

If there was a way that we could help the woman even at home, or even if she did come to the doctor’s office, avoid the need for the more expensive technology and use a simple, cheap test that would have a result for us in a few minutes that would be a valuable tool.

That is, if there was some way that we could determine who really would benefit from some follow-up and who really doesn’t need it that would be great.
